The Clark Health and Wellness Center is a new state of the art medical facility that will provide high quality, convenient and easy access to all patient's health and wellness services under one roof.
Offering access to urgent care, primary care physicians, specialty physicians, rehab, lab, and imaging services for patients in our communities.
HMH is seeking customer service oriented individuals to provide clinical and administrative care and to create the best possible patient experience.
The Health and Wellness Center will be a multispecialty experience for our patients and team members.
As a member of an interdisciplinary team, this position is responsible for occupational therapy evaluation, treatment planning, treatment implementation, documentation and patient/family education.
Performs other professional duties and participates in department/facility initiatives based on needs and opportunities.
Responsibilties:
A day in the life of a Occupational Therapist at Hackensack Meridian Health includes:
Prepares for the evaluation in accordance with department policies/procedures and standards of practice including obtaining patient history, exhibiting knowledge of patient diagnoses and researching unfamiliar diagnoses.
Selects and administers tests and measures.
Additionally, synthesizes evaluation results to formulate the diagnosis, goals, prognosis and plan of care.
Establishes treatment plan based on evaluation results, goals and best practice.
Collaborates with interdisciplinary team, patients and patient's support system.
Assesses and re-assesses need for modifications to plan of care and goals.
Identifies need for consultations and/or refers the patient to another health care professional or health care services.
Initiates and completes discharge plan.
Provides education/training to patient and/or patient support system.
Provides assessments for ADLs/IADLs; therapeutic activities (e.
g.
transfers, bed mobility); rehabilitation equipment prescription and training and community reintegrationProvides neurological interventions include but are not limited to handling techniques, neurodevelopmental training, seating and positioning, vision/perception, cognitive rehabilitation, neuromuscular re-education, spasticity management and behavioral management.
Provides musculoskeletal Interventions:
This may include but is not limited to, therapeutic exercise; manual therapy; joint mobilizations;Addresses issues which include but are not limited to self-esteem, self-awareness, stress management, interpersonal and social skills, and wellness.
Collects and records patient care data that follows format and guidelines of the patient care service, regulatory agencies and third-party payers which reflects an understanding of reimbursement systems and their impact on patient care.
This includes, but is not limited to, initial evaluation, daily notes, reassessments/progress reports, discharge plans and communication with members of the team.
Additionally records daily billing and follows department guidelines for productivity.
Other duties and/or projects as assigned.
Adheres to HMH's Organizational and Managerial competencies and standards of behavior.
Lifts a minimum of 5 lbs.

Qualifications:
Education, Knowledge, Skills and Abilities Required:
Graduate of an AOTA Occupational Therapy Program or equivalent.
Eligible for school certification Education, Knowledge, Skills and Abilities Preferred:
Experience preferred, entry level acceptable, brain injury experience a plus Computer skills and knowledge of applications such as word, excel power point preferred Licenses and Certifications Required:
Occupational Therapist License.
